Diabetes Management
Our Diabetes Management Project offers one-on-one support for disease management for individuals with the least access to care. Started in 1999, the Diabetes Management Program was targeted to low-income, minority Meals-On-Wheels participants who suffered from diabetes. Staff members help those with diabetes monitor their blood sugars, tell them about community resources and keep them on a schedule to get their eyes checked and visit the foot clinic.
Loaves & Fishes Centers has been successful in teaming with volunteers and staff from all areas of the aging and medical community to complete independent assessments and plans to assist individuals better manage their diabetes. These care plans are coordinated with the participant, case manager, personal physician and nutritionist.
Diabetes Support Groups are currently operating at our Ambleside and Lents Centers.
